Job Title

Physical Therapist – PACE Program

Location

Mishawaka, IN

Contract | Days | Onsite

Key Responsibilities

Provide physical therapy services to participants enrolled in a PACE (Program of All-inclusive Care for the Elderly) program
Conduct comprehensive physical therapy evaluations and develop individualized treatment plans
Deliver therapeutic interventions aimed at maintaining or improving participants' functional mobility and independence
Collaborate with an interdisciplinary care team to coordinate participant care and treatment goals
Document evaluations, progress notes, and treatment plans in accordance with regulatory and program requirements

Required Qualifications

Current, active Physical Therapist (PT) license in the state of Indiana
Graduate of an accredited Physical Therapy program
Experience providing physical therapy services in a clinical or rehabilitative setting
Ability to work effectively within an interdisciplinary team environment

Preferred Experience / Skills

Prior experience working with geriatric or elderly patient populations
Familiarity with PACE program models or all-inclusive care environments
