Key Responsibilities

• Evaluate patient function through formal and informal testing measures to assess skeletal, neurological, respiratory, and cardiovascular status.
• Develop goal-specific treatment regimens based on assessment data and patient tolerance in accordance with physical therapy standards and physician orders.
• Submit and maintain written treatment plans coordinated with the physician, updating the plan of care as needed to reflect changes in patient status.
• Devise frequency- and duration-specific treatment schedules in coordination with patients, families, caregivers, and clinical support staff.
• Educate patients, families, and caregivers regarding their condition, prognosis, treatment rationale, and interventions including exercise, modalities, and rest.
• Delegate appropriate responsibilities to Physical Therapist Assistants and Aides, co-signing verbal orders as necessary.

Required Qualifications

• Active Michigan Physical Therapist License (LARA verified, Active status required).
• Active Basic Life Support (BLS) certification through the American Heart Association.
• Outpatient physical therapy experience required.
• Current Skills Checklist pertinent to Physical Therapy (within the last year) required.
• Two supervisory reference forms completed within the last two years required.
• Candidates must reside outside a 100-mile radius of the facility location.

Preferred Experience / Skills

• Experience working with Epic EMR.
• Familiarity with rural or critical access hospital environments.
