Key Responsibilities

Evaluate and treat patients in a Skilled Nursing Facility (SNF) setting
Develop and implement individualized physical therapy treatment plans
Monitor and document patient progress toward established therapy goals
Collaborate with interdisciplinary care teams to coordinate patient care
Educate patients and families on home exercise programs and safe mobility techniques

Required Qualifications

Active Physical Therapist license in the state of Massachusetts
Graduate of an accredited Physical Therapy program
Current CPR/BLS certification
Experience working in a SNF or similar post-acute care environment

Preferred Experience / Skills

Prior experience with geriatric patient populations
Familiarity with MDS documentation and Medicare compliance in SNF settings
