Job Title

Physical Therapist – Swing Bed & Outpatient

Location

Kenmare, ND (approximately 45 miles from Minot, ND)

Schedule / Job Type

Contract | Full-Time | Days | 5 days/week, 40 hours/week | Onsite

Key Responsibilities

• Perform comprehensive physical therapy assessments and develop individualized, evidence-based treatment plans.
• Provide therapeutic interventions focused on mobility, strength, pain reduction, and functional independence.
• Supervise and coordinate care with Physical Therapist Assistants (PTAs).
• Collaborate with interdisciplinary team members to coordinate patient-centered care and discharge planning.
• Educate patients and caregivers on safe mobility techniques, equipment use, and home exercise programs.
• Evaluate, plan, implement, and document care in accordance with established standards of performance and organizational policies.

Required Qualifications

• Active Physical Therapist license in the state of North Dakota.
• Valid driver's license with reliable personal vehicle available for travel to patient locations.
• Ability to work across varied environments including patient homes, office settings, and community locations.

Preferred Experience / Skills

• Minimum of 2 years of experience as a licensed Physical Therapist.
• Experience working with geriatric populations in home health, hospice, or swing bed settings.
• Familiarity with interdisciplinary care team collaboration and discharge planning processes.
