Contract | Full-Time | Onsite | Day Shift | 5x8-hour shifts, 40 hours per week

Key Responsibilities

Provide direct physical therapy services to patients in an outpatient clinical setting
Evaluate, assess, and develop individualized treatment plans for patients
Deliver therapeutic interventions in accordance with established clinical protocols and standards of care
Complete required clinical documentation accurately and in a timely manner
Collaborate with interdisciplinary care teams to support optimal patient outcomes

Required Qualifications

Active Physical Therapist license — permanent Indiana state license required; compact license not accepted
Prior experience as a Physical Therapist in a relevant clinical setting
Ability to meet facility-specific credentialing and contract QA requirements prior to start
Willingness to complete any facility-required documentation outside of standard onboarding clearance processes

Preferred Experience / Skills

Experience in an outpatient physical therapy environment
Familiarity with outpatient documentation systems and clinical workflows
