Occupational Therapist – Home Health

Schedule / Job Type

Contract (13 Weeks) | Full-Time | Onsite | Monday–Friday, 8:00 AM–5:00 PM | Weekend/On-Call Requirements May Apply

Key Responsibilities

Provide occupational therapy services to patients in a home health setting
Conduct evaluations and develop individualized treatment plans to support patient functional goals
Document patient progress and clinical notes using HCHB electronic medical records software
Coordinate care with interdisciplinary home health team members
Travel within an assigned territory to deliver patient care
Participate in weekend and on-call rotations as determined

Required Qualifications

Active Occupational Therapist (OT) license in the state of Wisconsin
Previous home health experience required
Proficiency with HCHB (Homecare Homebase) documentation platform required
Valid driver's license and reliable transportation for territory-based patient visits

Preferred Experience / Skills

Experience managing a home health patient caseload across a defined geographic territory
Familiarity with on-call and weekend coverage rotations in a home health environment
