Key Responsibilities

• Provide direct physical therapy services to home health patients in accordance with each patient's individualized plan of care
• Collaborate with interdisciplinary team members to develop and update comprehensive patient care plans
• Evaluate and recommend rehabilitation equipment including splints, orthoses, prostheses, wheelchairs, and ADL equipment
• Apply appropriate therapeutic modalities as indicated by patient need and clinical assessment
• Complete and maintain accurate clinical documentation, including OASIS assessments, in a timely manner
• Travel independently to patient homes within the designated service area

Required Qualifications

• Active Massachusetts Physical Therapist license (must be provided at time of submission)
• Current Basic Life Support (BLS) certification through the American Heart Association
• Certified Home Health experience with demonstrated proficiency in OASIS documentation (required and non-negotiable)
• Minimum of 2 years of acute nursing care (hospital-based) and home care experience
• Valid driver's license and personal vehicle required for patient home visits
• Ability to work independently with minimal supervision

Preferred Experience / Skills

• Additional acute care experience beyond minimum requirements
• Experience working in a teaching hospital or large health system environment
