Contract | Days | Monday–Friday | 13-Week Assignment

Key Responsibilities

Evaluate and treat residents in a long-term care setting to restore or improve physical function and mobility
Develop and implement individualized physical therapy treatment plans based on patient assessments
Document patient progress, treatment plans, and outcomes in accordance with facility and regulatory standards
Collaborate with interdisciplinary care teams to ensure coordinated and effective patient care
Educate patients, families, and caregivers on therapeutic exercises and home programs

Required Qualifications

Active Physical Therapist (PT) license in the state of Minnesota or eligibility for licensure
Graduate of an accredited Physical Therapy program
Ability to work a Monday–Friday day shift schedule
Minimum 36 hours per week availability

Preferred Experience / Skills

Prior experience in a skilled nursing or long-term care environment
Familiarity with geriatric patient populations and care protocols
