Occupational Therapist – Home Health

Contract | Full-Time | Days (5x8 Hours, 8:00–16:30) | Onsite / Field-Based

Key Responsibilities

  • Provide skilled occupational therapy services to home health patients in Medford, OR and surrounding areas
  • Conduct patient evaluations and deliver individualized treatment plans in a 1:1 care setting
  • Visit up to 6 patients per day, meeting a standard productivity expectation of 30 points per day
  • Complete clinical documentation using EPIC and OASIS
  • Manage a patient caseload that may require travel within the service area, with an occasional radius of 50 or more miles

Required Qualifications

  • Minimum of 1 year of skilled home health occupational therapy experience required
  • Active or pending OT licensure in the state of Oregon
  • Basic Cardiac Life Support (BCLS) certification required
  • Proficiency with OASIS documentation is required
  • Reliable personal transportation required
  • COVID-19 primary vaccination series plus at least one booster required

Preferred Experience / Skills

  • Experience with EPIC electronic health records
  • Familiarity with productivity point systems in a home health environment
