Home Care Physical Therapist

Location

Barre, VT

Contract | Days | Onsite

Key Responsibilities

Assess patients' conditions and develop individualized plans of care to maximize mobility and independence
Collaborate with an interdisciplinary team to ensure patients' needs are met
Perform standard therapy measurement techniques including ROM, strength, and balance testing
Utilize a variety of therapeutic modalities to maximize care provided to patients in a home setting
Provide case management responsibilities as part of the home health care team

Required Qualifications

Active Vermont Physical Therapist licensure
Home Health experience required
OASIS experience required
Ability to provide one-on-one therapy care in a patient home setting

Preferred Experience / Skills

Experience with McKesson/Netsmart EMR system strongly preferred
Prior case management experience in a home health environment
