Key Responsibilities

• Complete comprehensive evaluations of assigned patients and develop, implement, and modify individualized treatment plans in accordance with patient needs, goals, and physician direction.
• Complete medical record documentation in a clear, concise, accurate, and timely manner including goal writing, functional quality indicator assessments, letters of medical necessity, and charge entry in compliance with state, federal, and intermediary guidelines.
• Delegate and direct discipline-specific therapy services for assigned patients, ensuring care is provided under signed, valid physician orders and within insurance approvals.
• Participate in student programs as assigned, providing supervision, developing learning objectives, and completing student evaluations.
• Adhere to discipline-specific State Practice Act and all applicable regulatory agency requirements, maintaining professional licensure in good standing.
• Contribute to organizational performance improvement initiatives and provide feedback through staff meetings, focus groups, or direct supervision channels.

Required Qualifications

• Active and unrestricted Physical Therapist licensure in the state of Indiana.
• Ability to perform comprehensive patient evaluations and develop evidence-based treatment plans.
• Proficiency in clinical documentation including progress notes, goal writing, and letters of medical necessity.
• Knowledge of and adherence to state and federal regulatory guidelines governing physical therapy practice.
• Commitment to patient and employee safety standards and safe patient handling practices.

Preferred Experience / Skills

• Prior experience in an outpatient physical therapy setting.
• Experience supervising or mentoring physical therapy students or clinical affiliates.
• Active involvement in continuing education and professional development activities.
