Contract | Full-Time | Onsite | Day Shift | 5 x 8-hour shifts per week, 40 hours per week

Key Responsibilities

Provide direct physical therapy services to patients in an outpatient or clinical setting
Evaluate, assess, and develop individualized treatment plans for patients
Deliver hands-on therapeutic interventions in accordance with plan of care
Complete required clinical documentation accurately and in a timely manner
Collaborate with interdisciplinary team members to support patient progress and outcomes

Required Qualifications

Active Physical Therapist license (permanent state license required)
Experience in a physical therapy clinical setting
Ability to meet credentialing and contract quality assurance requirements prior to start
Must maintain compliance with all facility onboarding and documentation requirements

Preferred Experience / Skills

Prior experience working in an outpatient physical therapy environment
Familiarity with clinical documentation systems and electronic health records
