Contract | Full-Time | Days | Onsite | Monday–Friday (flexible scheduling options available including Tuesday–Saturday or Sunday–Thursday)
Key Responsibilities
- Provide physical therapy services across both Skilled Nursing Facility (SNF) and Home Health (HH) settings, with approximately 50% of time in each setting on a week-to-week basis
- Conduct evaluations and deliver ongoing therapy treatment in accordance with productivity expectations (SNF: 82%; Home Health: 95% inclusive of drive time and documentation)
- Supervise Physical Therapist Assistants (PTAs) as needed
- Document patient care using Net Health and HCHB software platforms
- Travel between patient locations within the Home Health setting; mileage and drive time between locations is compensated
- May be requested to provide coverage at additional nearby locations to support staffing needs
Required Qualifications
- Active Physical Therapist (PT) license in the state of Nebraska or eligibility to obtain licensure
- Ability to work in both SNF and Home Health environments
- Proficiency with or ability to learn electronic documentation systems (Net Health and HCHB)
- Ability to meet setting-specific productivity standards
- Valid driver's license and reliable transportation for Home Health patient visits
Preferred Experience / Skills
- Prior experience in a Skilled Nursing Facility and/or Home Health setting
- Experience supervising Physical Therapist Assistants
- Familiarity with points-based Home Health visit structures and visit documentation requirements
