Contract | Full-Time | Days | Onsite | Monday–Friday (flexible shifted schedules available: Tue–Sat or Sun–Thu) | 36–40 hours per week
Key Responsibilities
- Administer and perform therapeutic interventions across SNF and Home Health settings, including evaluations, group therapy, and concurrent treatment
- Assess clients' needs and develop individualized plans of care that promote independence, choice, and quality of life
- Document observations, services provided, and clinical notes using Net Health and HCHB software
- Communicate and coordinate with physicians and interdisciplinary team members regarding changes in client status and care orders
- Train clients, family members, and staff on therapeutic interventions and equipment use
- Supervise assigned Certified Occupational Therapy Assistants (COTAs) in accordance with applicable Federal and State regulations
Required Qualifications
- Graduate of an accredited Occupational Therapy program
- Active OT licensure or eligibility for licensure in the state of Nebraska
- Ability to meet SNF productivity expectation of 82% and Home Health productivity expectation of 90% (inclusive of drive time and documentation)
- Ability to lift up to 50 lbs. and perform the physical demands of the role
- Must be at least 18 years of age and able to communicate effectively in English, both orally and in writing
Preferred Experience / Skills
- Prior experience in a rehabilitation or geriatrics setting
- Experience working in SNF and/or Home Health environments
- Familiarity with Net Health and/or HCHB documentation software
