Job Title

Physical Therapist – Home Health

Location

Contract | Part-Time | Days | Onsite

Key Responsibilities

Plan and conduct segments of treatment programs designed to restore and improve physical, social, and mental functions in a home health setting
Complete evaluations to ensure quality standards and address assessment areas requiring attention
Establish individualized treatment plans using appropriate clinical techniques, tools, and protocols
Develop individualized care and discharge plans and communicate them to appropriate parties
Complete comprehensive and accurate clinical documentation within established deadlines
Supervise assigned therapy assistants and serve as co-leader in an interdisciplinary approach to patient care

Required Qualifications

Active Hawaii state Physical Therapist license
Minimum of 2 years of Physical Therapist experience
Minimum of 1 year of home health Physical Therapy experience
Bachelor's degree in Physical Therapy or related field
Experience with Epic EMR
Current CPR and BLS (AHA) certification

Preferred Experience / Skills

Master's degree in Physical Therapy or related field
Prior travel therapy assignment experience
