Key Responsibilities

Provide outpatient physical therapy services to an average of 14–16 patients per day
Evaluate patients and develop individualized treatment plans to address functional deficits and rehabilitation goals
Deliver evidence-based therapeutic interventions in an outpatient clinical setting
Document patient progress, treatment plans, and clinical notes in accordance with facility and regulatory standards
Collaborate with interdisciplinary care team members to ensure coordinated patient care

Required Qualifications

Active Physical Therapist (PT) license in the state of Wisconsin or eligibility to obtain licensure
Graduate of an accredited Physical Therapy program
Current CPR/BLS certification
Experience working in an outpatient physical therapy setting

Preferred Experience / Skills

Demonstrated ability to manage a high patient volume in an outpatient environment
Strong documentation and time management skills
