Occupational Therapist (OT)

Schedule / Job Type

Contract | Days | Full-Time (35 hours guaranteed per week)

Key Responsibilities

Provide occupational therapy services to patients across multiple care locations
Evaluate patients and develop individualized treatment plans to support functional independence
Implement therapeutic interventions in alignment with facility protocols and patient goals
Coordinate care and communicate effectively across multiple facility settings
Document patient progress and maintain accurate clinical records in accordance with regulatory standards

Required Qualifications

Active Occupational Therapist (OT) license in the state of Illinois
Graduate of an accredited Occupational Therapy program
NBCOT certification (or eligibility)
Ability to work across multiple facility locations as required

Preferred Experience / Skills

Prior experience working in a multi-site or split-facility setting
Strong organizational and time management skills to manage caseloads across locations
