Contract | Part-Time | Rotating Shift | Onsite | Tuesday & Thursday (10-hour days), Friday 12:00 PM – 6:00 PM, Saturday (10-hour day), Sunday 8:00 AM – 12:00 PM; Mondays & Wednesdays off

Key Responsibilities

• Provide physical therapy services in a hospital-based outpatient setting
• Deliver inpatient acute and swing bed physical therapy care as needed
• Evaluate patients and develop individualized treatment plans
• Implement and progress therapeutic interventions in alignment with patient goals
• Collaborate with interdisciplinary care teams within a Critical Access Hospital environment
• Maintain accurate and timely clinical documentation

Required Qualifications

• Minimum 2 years of current Physical Therapy experience (new graduates will not be considered)
• Outpatient Physical Therapy experience required
• Active New Hampshire Physical Therapist license, current and in hand at time of submission
• Active American Heart Association (AHA) Basic Life Support (BLS) certification, current and in hand at time of submission
• Completed Physical Therapy skills checklist
• One supervisory signed reference from within the past year of employment

Preferred Experience / Skills

• Inpatient acute care Physical Therapy experience highly preferred
• Experience working within a Critical Access Hospital or rural healthcare setting
