Key Responsibilities
- Complete comprehensive evaluations of assigned patients and develop, implement, and modify individualized treatment plans in accordance with patient needs, goals, and physician direction
- Complete medical record documentation in a clear, concise, accurate, and timely manner including goal writing, functional quality indicator assessments, letters of medical necessity, and charge entry in compliance with state, federal, and intermediary guidelines
- Delegate and direct all discipline-specific therapy services for assigned patients, ensuring functional treatment programs support effective return to community activities
- Participate in student programs as assigned, providing appropriate supervision, developing learning objectives, and completing student evaluations
- Adhere to discipline-specific State Practice Act and all applicable regulatory requirements, maintaining professional licensure in good standing
- Actively engage in professional development through continuing education, self-study, and involvement in professional organizations and activities
Required Qualifications
- Active and unrestricted Physical Therapist license in the state of Arizona
- Experience in outpatient orthopedic physical therapy
- Proficiency in clinical documentation, goal writing, and compliance with state and federal guidelines
- Demonstrated commitment to patient and employee safety standards
- Ability to work under signed, valid physician orders and within insurance authorization requirements
Preferred Experience / Skills
- Experience supervising or mentoring physical therapy students in a clinical setting
- Familiarity with performance improvement methodologies and quality initiatives in a rehabilitation environment
- Experience with functional quality indicator assessments and letters of medical necessity
