Occupational Therapist – Home Health
Location
Portland, OR
Schedule / Job Type
Contract | Days (5x8 Hours, 8:00–16:30) | Onsite / Field-Based
Key Responsibilities
- Provide occupational therapy services to home health patients in the West Portland area and surrounding communities
- Conduct patient evaluations and deliver skilled therapeutic interventions in a 1:1 home setting, visiting up to 6 patients per day
- Complete Oasis assessments accurately and in accordance with home health standards
- Document patient care using EPIC Remote Client and Rover mobile application
- Manage a daily caseload while minimizing drive time across a service area with an occasional maximum radius of approximately 60 miles per day
Required Qualifications
- Active Oregon Occupational Therapist license
- Minimum of 1 year of skilled home health experience
- Oasis experience required
- Basic Cardiac Life Support (BCLS) certification
- Primary series COVID-19 vaccination required
- Reliable personal transportation required
Preferred Experience / Skills
- Familiarity with EPIC Remote Client for home health charting
- Experience meeting productivity point expectations in a home health environment (standard expectation of 30 points per day)
