Key Responsibilities
- Complete comprehensive evaluations of assigned patients and develop, implement, and modify individualized treatment plans in accordance with patient needs and physician direction.
- Collaborate with the interdisciplinary team to address barriers to discharge, including equipment trialing and ordering, patient and family training, home exercise program issuance, and planning for continuing services.
- Manage individual, concurrent, and group treatment sessions effectively based on patient care needs, demonstrating flexibility in caseload management.
- Complete medical record documentation in a clear, concise, accurate, and timely manner including daily notes, goal writing, functional quality indicator assessments, letters of medical necessity, discharge documentation, and charge entry in compliance with state, federal, and intermediary guidelines.
- Participate in student programs as assigned, providing appropriate supervision, developing learning objectives, and completing student evaluations.
- Actively participate in professional development, performance improvement initiatives, and patient and professional advocacy efforts.
Required Qualifications
- Active and unrestricted Physical Therapist licensure in good standing in the state of Arizona.
- Adherence to the state Physical Therapy Practice Act and all applicable regulatory agency requirements.
- Completion of all state-mandated continuing education requirements.
- Demonstrated ability to perform comprehensive patient evaluations and develop clinically reasoned treatment plans with discharge-focused goals.
- Ability to complete timely and accurate medical record documentation in compliance with applicable standards and policies.
Preferred Experience / Skills
- Experience working in an inpatient rehabilitation or acute care hospital setting.
- Familiarity with interdisciplinary team collaboration and discharge planning processes.
- Experience supervising or mentoring Physical Therapy students in a clinical setting.
