Key Responsibilities

• Provide physical therapy evaluation and treatment services within a skilled nursing facility (SNF) setting
• Develop and implement individualized treatment plans to support patient recovery and functional goals
• Document patient progress and maintain accurate clinical records in compliance with facility and regulatory standards
• Collaborate with interdisciplinary care teams to ensure coordinated patient care
• Float between assigned worksites as operationally needed

Required Qualifications

• Active Physical Therapist license in the state of California
• Minimum of 1 year of experience in a skilled nursing facility (SNF) setting
• Ability and willingness to float to additional worksites as required
• Completion of standard credentialing requirements

Preferred Experience / Skills

• Experience using Casamba documentation software
• Familiarity with SNF regulatory and compliance standards
