Key Responsibilities
- Complete comprehensive evaluations of assigned patients and develop, implement, and modify individualized treatment plans in collaboration with physician direction and the interdisciplinary team.
- Manage individual, concurrent, and group treatment sessions; ensure plan of care is communicated, progressed, and aligned with discharge goals throughout the length of stay.
- Complete all medical record documentation in a clear, concise, accurate, and timely manner, including daily notes, goal writing, functional quality indicator assessments, letters of medical necessity, and discharge documentation in compliance with state, federal, and intermediary guidelines.
- Provide patient and family education, including home exercise program instruction, equipment trialing and ordering, and planning for continuation of care post-discharge.
- Participate in student programs as assigned, providing appropriate supervision, developing learning objectives, and completing student evaluations.
- Actively engage in professional development, performance improvement initiatives, and adherence to discipline-specific state practice acts and regulatory requirements.
Required Qualifications
- Active and unrestricted Physical Therapist license in the state of Arizona.
- Ability to perform comprehensive patient evaluations and develop clinically reasoned treatment plans.
- Knowledge of and adherence to state and federal regulatory guidelines governing physical therapy practice.
- Demonstrated competency in safe patient handling and compliance with safety policies and procedures.
- Ability to complete timely and accurate medical record documentation using approved formats.
Preferred Experience / Skills
- Experience working in an inpatient rehabilitation or acute care hospital setting.
- Familiarity with interdisciplinary team collaboration and discharge planning processes.
- Experience supervising or mentoring physical therapy students in a clinical setting.
