Key Responsibilities

• Provide physical therapy services to patients in their place of residence who have physical impairments, functional limitations, or disabilities resulting from injury, disease, or other causes
• Serve a diverse patient population ranging in age from birth to geriatric
• Collaborate with patients, families, physicians, and allied health professionals to identify needs and coordinate care
• Maintain thorough and timely electronic documentation in accordance with agency policies, procedures, and standards of practice
• Communicate effectively across interdisciplinary care teams to ensure coordinated service delivery
• Travel to assigned patient cases within the designated service area

Required Qualifications

• Active Physical Therapist professional license in the state of Massachusetts
• Minimum of 1 year of travel therapy experience
• Minimum of 1 year of home care experience
• Ability to travel to patient cases as assigned

Preferred Experience / Skills

• Experience working with patients across a wide age range, including pediatric and geriatric populations
• Familiarity with electronic medical records and documentation platforms used in home care settings
