Key Responsibilities

Provide direct physical therapy services to patients in an outpatient setting
Complete all required documentation accurately and in a timely manner
Develop and implement individualized treatment plans based on patient evaluations
Collaborate with interdisciplinary team members to support patient progress and outcomes
Maintain compliance with facility credentialing and quality assurance requirements

Required Qualifications

Active Physical Therapist license required
Experience in an outpatient physical therapy setting
Must meet contract credentialing and quality assurance requirements prior to start

Preferred Experience / Skills

Prior experience working in a high-volume outpatient therapy environment
Strong documentation and organizational skills
