Key Responsibilities
- Provide physical therapy services primarily within a skilled nursing facility setting, with the ability to float to an inpatient acute care hospital as needed
- Treat approximately 8–9 post-acute patients per day across assigned stations (A, B, C, and D)
- Manage a caseload including patients with total joint replacements (TKA, THA), CVA/stroke, COPD, pneumonia, deconditioning, and balance difficulties associated with general medical conditions
- Work independently to perform patient evaluations, treatment planning, and transfers
- Collaborate closely with an interdisciplinary team to coordinate patient care
- Document patient care accurately using Cerner EMR
Required Qualifications
- Active California Physical Therapist (PT) license
- AHA Basic Life Support (BLS) certification
- Ability to work independently and perform patient transfers
- Strong communication skills and ability to function effectively within an interdisciplinary team
Preferred Experience / Skills
- Experience in a skilled nursing facility setting strongly preferred; acute care experience also considered
- New graduates with a minimum of 6 months of experience in skilled nursing or acute care will be considered
- Familiarity with geriatric, orthopedic, and neurological patient populations
- Prior experience with Cerner EMR
