Key Responsibilities

• Provide physical therapy services to patients in their place of residence who have physical impairments, functional limitations, or disabilities resulting from injury, disease, or other causes
• Serve a diverse patient population ranging in age from birth to over 100 years
• Collaborate with patients, families, physicians, and allied health professionals to identify needs and coordinate care
• Maintain thorough and timely electronic documentation in accordance with agency policies, procedures, and standards of practice
• Travel to patient cases as assigned across the designated service area
• Participate in weekend rotations on a scheduled basis (approximately once every six weeks)

Required Qualifications

• Active Physical Therapist professional license in the state of Massachusetts required
• Minimum of 1 year of experience within a certified home health agency (certified home health experience only — hospice, private care, or general case management experience does not satisfy this requirement)
• Minimum of 1 year of case management experience within a certified home health setting
• Ability to travel to patient cases as assigned

Preferred Experience / Skills

• Experience with interdisciplinary team communication and coordination
• Proficiency with electronic medical records and documentation platforms
• Comfort working with a broad patient demographic, including pediatric and geriatric populations
