Key Responsibilities
- Provide direct physical therapy services to patients in accordance with physician orders and plan of care
- Evaluate, assess, and develop individualized treatment plans to meet patient goals
- Document patient progress and maintain accurate clinical records
- Wear a facility-issued photo badge at all times while on site
- Collaborate with interdisciplinary team members to support patient outcomes
Required Qualifications
- Active Physical Therapist (PT) license in the state of Washington
- Graduate of an accredited Physical Therapy program
- Must present a valid state or federal issued photo identification upon arrival at the facility
- Minimum 36-hour weekly availability guaranteed
Preferred Experience / Skills
- Prior experience in a skilled nursing or long-term care setting
- Strong documentation and communication skills
- Ability to adapt quickly in a high-need clinical environment
