Key Responsibilities

Provide occupational therapy services to patients within a home health setting across a designated service area
Maintain a minimum productivity expectation of 30 points per week
Complete all clinical documentation by 10:00 AM the following day
Utilize HCHB (Homecare Homebase) for documentation and care coordination
Travel to patient homes within the assigned coverage radius

Required Qualifications

Active Occupational Therapist (OT) license in the state of Washington
Minimum of one (1) year of home health experience required
Proficiency with HCHB (Homecare Homebase) EMR system
Must be available to cover a 50+ mile radius from the office location

Preferred Experience / Skills

Experience managing a high-volume caseload in a home health environment
Strong time management and documentation skills to meet next-day charting requirements
