Key Responsibilities

• Complete comprehensive evaluations of assigned patients and develop, implement, and modify individualized treatment plans in accordance with patient needs, goals, and physician direction
• Maintain accurate, timely medical record documentation including goal writing, functional quality indicator assessments, letters of medical necessity, and charge entry in compliance with state, federal, and payer guidelines
• Delegate and direct discipline-specific therapy services for assigned patients, ensuring functional treatment programs support effective return to community activities
• Serve as an educator for patients, families, students, and coworkers; participate in student programs by providing supervision, developing learning objectives, and completing student evaluations
• Adhere to discipline-specific State Practice Act and all applicable regulatory requirements; maintain professional licensure in good standing and fulfill all state-mandated continuing education requirements
• Contribute to organizational performance improvement initiatives and apply learned information to meet strategic objectives

Required Qualifications

• Active Physical Therapist license in good standing (Arizona licensure required or ability to obtain)
• Compliance with discipline-specific professional code of conduct and ethical standards
• Ability to complete documentation in compliance with state, federal, and intermediary guidelines
• Commitment to patient and employee safety practices in all aspects of care delivery

Preferred Experience / Skills

• Prior experience in an outpatient clinical setting
• Experience supervising or mentoring physical therapy students
• Demonstrated commitment to ongoing professional development through continuing education and professional organizations
