Key Responsibilities

• Provide direct physical therapy services to patients in an outpatient setting
• Complete required documentation accurately and in a timely manner in accordance with facility and compliance standards
• Evaluate patients and develop individualized treatment plans to address functional deficits
• Deliver evidence-based therapeutic interventions to support patient recovery and rehabilitation goals
• Collaborate with interdisciplinary team members to coordinate patient care

Required Qualifications

• Active Physical Therapist license required; compact license accepted but permanent state license must be obtained promptly upon assignment start
• Experience in an outpatient physical therapy setting
• Must meet credentialing and contract QA requirements prior to start
• Candidates who have worked permanently or PRN at this facility within the last year are not eligible for this assignment

Preferred Experience / Skills

• Prior experience working in a high-volume outpatient rehabilitation environment
• Strong documentation skills and familiarity with electronic medical records
• Ability to work independently while maintaining collaborative team relationships
