Key Responsibilities
- Provide skilled physical therapy services to home health patients in Seaside, OR and surrounding areas
- Manage a caseload of up to 6 patients per day, maintaining standard productivity expectations
- Complete OASIS assessments as required for home health documentation
- Chart using EPIC Remote Client for Home Health and a mobile phone application
- Travel independently to patient homes; occasional daily drive radius of up to approximately 60 miles
Required Qualifications
- Active Physical Therapist license valid in Oregon (compact license must be in hand at time of submission; pending licenses considered)
- Minimum of 1 year of skilled home health experience required
- Basic Cardiac Life Support (BCLS) certification required
- OASIS proficiency required
- Must have reliable personal transportation
- COVID-19 primary vaccination series plus one booster required
- Locals accepted; candidates within a 50–75 mile radius of the facility are considered local and are welcome to apply
Preferred Experience / Skills
- Experience with EPIC electronic health records platform
- Familiarity with home health productivity standards and point-based documentation systems
- Ability to manage an independent schedule with minimal supervision in a community-based setting
