Key Responsibilities
- Provide physical therapy services to patients across an assigned multi-city territory in the Contra Costa County region
- Conduct evaluations, re-evaluations, OASIS assessments, routine visits, and discharge visits in a home health setting
- Manage an active patient caseload with an expectation of at least 27 points per week under guarantee arrangements
- Complete all required clinical documentation, including visit notes and OASIS documentation, in a timely manner
- Travel within and, when necessary, outside the assigned service area to meet patient visit expectations
- Weekend coverage may be required; must be willing to see COVID-positive and COVID PUI patients
Required Qualifications
- Current California Physical Therapist (PT) License
- BLS Certification
- Valid Driver's License, current auto insurance, and current vehicle registration
- Up-to-date health and immunization documentation: TB (annual), MMR, Varicella, HepB, TDap (within 10 years), and Flu (annual)
- Must pass a drug screen and physical examination prior to assignment
Preferred Experience / Skills
- Prior experience in a home health or field-based physical therapy setting
- Familiarity with OASIS documentation and home health visit workflows
- Ability to manage a high-volume caseload independently across a wide geographic territory
