Key Responsibilities
- Conduct comprehensive patient evaluations and develop, implement, and modify individualized treatment plans in accordance with patient needs, goals, and physician direction
- Conduct 60-minute initial evaluations followed by 30-minute follow-up sessions
- Complete medical record documentation in a clear, concise, accurate, and timely manner, including goal writing, functional quality indicator assessments, letters of medical necessity, and charge entry in compliance with state, federal, and payer guidelines
- Design functional treatment programs that support patients' effective return to community activities
- Delegate and direct discipline-specific therapy services for assigned patients
- Serve as an educator for patients, families, students, and coworkers; participate in student programs as assigned, including supervision, development of learning objectives, and completion of student evaluations
Required Qualifications
- Active Physical Therapist license in good standing in the state of Colorado
- Adherence to the Colorado Physical Therapy Practice Act and all applicable regulatory requirements
- Completion of all state-mandated continuing education requirements
- Ability to ensure care is provided under signed, valid physician orders and within insurance approvals
Preferred Experience / Skills
- Prior experience in an outpatient physical therapy setting
- Experience with functional quality indicator assessments and letters of medical necessity
- Demonstrated commitment to professional development through continuing education and involvement in professional organizations
