Key Responsibilities
- Provide occupational therapy evaluations and treatments to residents in a skilled nursing facility setting
- Develop and implement individualized treatment plans to support patient functional independence and rehabilitation goals
- Document patient progress, treatment plans, and outcomes in accordance with facility and regulatory standards
- Collaborate with interdisciplinary care teams to ensure comprehensive patient care
- Adhere to a 14-day notice policy as outlined in the contract terms
Required Qualifications
- Active Occupational Therapist license required
- Skilled Nursing Facility (SNF) experience required
- Ability to work day shift, 8-hour shifts
Preferred Experience / Skills
- Experience with geriatric and long-term care patient populations
- Strong documentation and interdisciplinary communication skills
