Key Responsibilities

• Provide physical therapy services across multiple care settings including outpatient, inpatient, swing-bed, and home health
• Evaluate patients and develop individualized treatment plans to address functional deficits and rehabilitation goals
• Deliver evidence-based therapeutic interventions to promote recovery and improve patient outcomes
• Document patient progress, treatment plans, and clinical notes in accordance with facility and regulatory standards
• Collaborate with interdisciplinary care teams to coordinate patient care across settings

Required Qualifications

• Active Physical Therapist (PT) license in the state of New Mexico or eligibility to obtain
• Graduate of an accredited Physical Therapy program
• Ability to work effectively across outpatient, inpatient, swing-bed, and home health settings
• Expected 40 hours per week availability

Preferred Experience / Skills

• Prior experience in a hospital-based or multi-setting therapy environment
• Experience with swing-bed and home health patient populations
• Strong documentation and time-management skills
