Contract | Full-Time | Onsite/Field | Monday–Friday, 8:00 a.m. – 4:30 p.m.

Key Responsibilities

• Conduct home visits throughout the assigned service area, with primary coverage in Baltimore City and support for Baltimore County, seeing approximately 6 patients per day
• Complete assessments, evaluations, reassessments, plans of care, and discharge documentation including OASIS documentation
• Provide gait training, mobility assistance, transfer training, assistive device instruction, and home exercise program development
• Deliver orthopedic and neurologic rehabilitation interventions appropriate to an adult and geriatric patient population
• Provide patient and family education focused on safety and injury prevention
• Maintain expected productivity of 30 points per week and document care using the Kinnser/WellSky EMR system

Required Qualifications

• Active Maryland Physical Therapist license required upon start; candidates may submit with license pending
• BLS certification required
• Minimum 2 years of Physical Therapist experience required
• Home Health Physical Therapy experience required; first-time travelers are not accepted
• OASIS documentation experience required
• EMR experience required
• Must be comfortable traveling throughout the assigned service area to complete home visits

Preferred Experience / Skills

• Experience with Kinnser/WellSky EMR platform
• Strong background in both orthopedic and neurologic rehabilitation settings
• Experience working with adult and geriatric patient populations in a home-based care environment
