Contract | Full-Time | Days | 5x8-hour shifts | Expected 40 hours/week (32 hours during holiday weeks)

Key Responsibilities

• Provide direct physical therapy services to patients within an outpatient setting
• Complete required documentation in accordance with facility and credentialing standards prior to and throughout the assignment
• Deliver evidence-based therapeutic interventions aligned with patient goals and plan of care
• Maintain compliance with all credentialing, licensure, and facility-specific requirements

Required Qualifications

• Active Physical Therapist license — must obtain a permanent Oregon state license promptly; compact licensure may be accepted at start
• Prior experience in an outpatient physical therapy setting
• Ability to meet facility credentialing and contract QA requirements before assignment start
• Candidates who have worked at this facility (permanent or PRN) within the past year are not eligible for this assignment

Preferred Experience / Skills

• Previous travel therapy experience
• Familiarity with outpatient orthopedic or sports rehabilitation settings
