Key Responsibilities

Evaluate and treat patients in a skilled nursing facility setting
Develop and implement individualized physical therapy treatment plans
Monitor and document patient progress toward functional goals
Collaborate with interdisciplinary care teams to support patient outcomes
Maintain accurate and timely clinical documentation in accordance with facility standards

Required Qualifications

Active Physical Therapist (PT) license in the state of North Dakota or eligibility to obtain
Graduate of an accredited Physical Therapy program
Ability to begin assignment in October
Expected 36 hours per week

Preferred Experience / Skills

Prior experience in a skilled nursing facility (SNF) setting
Familiarity with MDS/PDPM documentation processes
Strong communication and patient-centered care skills
