Key Responsibilities
- Provide occupational therapy services to home health patients across an assigned caseload in the Concord and Walnut Creek areas
- Actively manage your caseload to maintain expected productivity, targeting a minimum of 30 points per week
- Proactively backfill cancelled visits by contacting patients to reschedule or adjust upcoming appointments to maintain a full schedule
- Complete clinical documentation accurately and in a timely manner using HCHB (HomeCare HomeBase)
- Collaborate with a supportive home office team to coordinate patient care and scheduling
Required Qualifications
- Active Occupational Therapist (OT) license in the state of California
- Minimum of 1–2 years of occupational therapy experience
- Proficiency with or ability to quickly learn HCHB (HomeCare HomeBase) documentation software
- Valid driver's license and reliable transportation for patient home visits
Preferred Experience / Skills
- Prior home health OT experience strongly preferred
- Prior experience with HCHB documentation platform
- Ability to self-manage schedule and work independently in a field-based setting
