Key Responsibilities

• Complete comprehensive evaluations of assigned patients and develop, implement, and modify individualized treatment plans in alignment with patient needs, goals, and physician direction
• Conduct 60-minute evaluations and 30-minute follow-up sessions; progress patients toward discharge within an established length of stay
• Complete medical record documentation in a clear, concise, accurate, and timely manner, including goal writing, functional quality indicator assessments, letters of medical necessity, and charge entry in compliance with state, federal, and payer guidelines
• Delegate and direct discipline-specific therapy services for assigned patients; ensure care is provided under signed, valid physician orders and within insurance approvals
• Participate in student programs as assigned, providing supervision, developing learning objectives, and completing student evaluations
• Adhere to discipline-specific State Practice Act and all applicable regulatory requirements; uphold professional standards and codes of conduct

Required Qualifications

• Active Physical Therapist license in good standing in the state of Colorado (or eligibility to obtain)
• Completion of all state-mandated continuing education requirements as required for licensure maintenance
• Demonstrated ability to perform comprehensive patient evaluations and develop evidence-based treatment plans
• Proficiency in clinical documentation, including goal writing and compliance with state, federal, and intermediary guidelines
• Commitment to patient and employee safety standards and adherence to clinic policies and procedures

Preferred Experience / Skills

• Prior experience in an outpatient physical therapy setting
• Experience supervising or mentoring PT students in a clinical environment
• Active involvement in professional development activities, continuing education, or professional organizations
• Familiarity with performance improvement methodologies and quality initiatives
