Key Responsibilities

Evaluate and treat patients with speech, language, voice, and swallowing disorders in a skilled nursing facility setting
Develop and implement individualized treatment plans based on patient needs and goals
Monitor and document patient progress, adjusting plans of care as appropriate
Collaborate with interdisciplinary care team members to support optimal patient outcomes
Provide education and guidance to patients, families, and caregivers regarding treatment strategies

Required Qualifications

Active Speech Language Pathologist license in the state of Indiana
Certificate of Clinical Competence in Speech-Language Pathology (CCC-SLP) from ASHA
Experience working in a skilled nursing facility or similar post-acute care environment
Strong clinical documentation and communication skills

Preferred Experience / Skills

Prior travel or contract therapy experience
Familiarity with MDS and skilled nursing facility documentation requirements
Experience with dysphagia evaluation and treatment protocols
