Key Responsibilities

• Provide physical therapy services to patients within a home health setting across the Skagit and North Snohomish coverage area
• Conduct patient evaluations, develop individualized treatment plans, and deliver skilled therapy interventions in the home environment
• Document all patient care activities and progress accurately using HCHB (Homecare Homebase) software
• Coordinate with the interdisciplinary care team to ensure continuity and quality of patient care
• Manage a caseload effectively across an assigned geographic territory, including travel between patient homes

Required Qualifications

• Active Physical Therapist (PT) license in the state of Washington
• Minimum of 1 year of home health experience required
• HCHB (Homecare Homebase) experience required
• Valid driver's license and reliable transportation for patient home visits

Preferred Experience / Skills

• Familiarity with the Skagit and North Snohomish County areas
• Experience managing a geographically dispersed home health caseload
• Strong time management and independent decision-making skills in a field-based setting
