Key Responsibilities
- Provide home health physical therapy services to an adult and geriatric patient population across Aiken, Barnwell, and Edgefield counties in South Carolina
- Manage a caseload of approximately 6 visits per day, working toward a full-time productivity target of around 25 units
- Treat patients with conditions including joint replacements, COPD, CHF, and Type 2 Diabetes
- Document patient care using Home Care Home Base (HCHB); system is expected to transition to Epic around September 2026
- Track mileage through HCHB/Epic; mileage is reimbursed at the IRS standard rate
- Adhere to holiday work expectations: if one or two major holidays fall within the assignment period, the clinician is expected to work those holidays; if three fall within the assignment period, the clinician is expected to work two
Required Qualifications
- Active South Carolina PT license or Compact PT license with South Carolina as an eligible member state
- BLS certification required
- Minimum of 1 year of experience preferred; first-time travelers considered
- Must complete all compliance documents by the Tuesday prior to the assignment start date; failure to comply may result in a two-week start delay due to bi-weekly orientation scheduling
- Clinicians without prior HCHB experience will be required to complete an online HCHB course before starting
- All requested time off (maximum 7 days per contract) must be finalized at time of submission; no schedule changes or RTO requests will be accepted after offer or during orientation week

Preferred Experience / Skills
- Prior home health physical therapy experience
- Experience with Home Care Home Base (HCHB) documentation system
- Familiarity with post-surgical joint replacement rehabilitation and management of chronic conditions such as COPD, CHF, and Type 2 Diabetes
