Key Responsibilities

Travel to patients' homes in the Brookings area to deliver physical therapy services
Implement individualized treatment plans under the direction of a supervising Physical Therapist
Monitor and document patient progress, adjusting interventions as appropriate
Communicate effectively with the care team regarding patient status and goals

Required Qualifications

Active PTA license in the state of Oregon (or eligibility to obtain)
Ability to travel independently to patient homes within the Brookings area
Valid driver's license and reliable transportation

Preferred Experience / Skills

Previous in-home or home health therapy experience
Strong organizational and time-management skills for managing a home-visit caseload
