Contract | Full-Time | Days | Monday–Friday, 8-hour shifts | Expected 40 hours/week

Key Responsibilities

• Travel to patients' homes within the local area to deliver physical therapy services
• Evaluate patients and develop individualized treatment plans to address functional goals
• Implement therapeutic interventions and monitor patient progress throughout the course of care
• Collaborate with patients, families, and care teams to support positive outcomes
• Maintain accurate and timely clinical documentation in accordance with regulatory standards

Required Qualifications

• Active Physical Therapist (PT) license in the state of Oregon or eligibility to obtain
• Graduate of an accredited Physical Therapy program
• Valid driver's license and reliable transportation for home visits

Preferred Experience / Skills

• Previous in-home or home health therapy experience strongly preferred
• Ability to work independently and manage a patient caseload in a community setting
