Physical Therapist (PT) – Travel Contract
Location
Lake Havasu City, AZ | Outpatient

Contract | 13 Weeks | Days | Monday–Friday, 8-hour shifts | Start Time: 8:00 AM
Key Responsibilities
- Provide outpatient physical therapy evaluations and treatments in a hospital-based outpatient setting
- Develop and implement individualized patient care plans to support functional recovery and rehabilitation goals
- Document patient progress and clinical findings accurately using the facility's EMR system (Medhost)
- Collaborate with interdisciplinary care teams to ensure coordinated, high-quality patient outcomes
- May be required to float to the Medical-Surgical unit as needed
Required Qualifications
- Active Physical Therapist (PT) license valid in the state of Arizona or eligibility to obtain
- Graduate of an accredited Physical Therapy program
- Ability to work Monday through Friday day shifts in an outpatient hospital environment
- Compliance with facility dress code (teal scrubs)
Preferred Experience / Skills
- 2 or more years of physical therapy experience
- Prior experience with Medhost EMR
- Experience in an outpatient or hospital-based therapy setting
