Contract | Part-Time | Days | Expected 20 hours/week (2–3 days)

Key Responsibilities

• Provide skilled speech-language pathology services to a geriatric home health caseload on a 1:1 basis
• Conduct patient visits in a home health setting, with up to 6 patients visited per day
• Meet productivity expectations of 30 points per full week (minimum 15 points for a 20-hour week)
• Utilize the HCHB electronic medical records system for documentation and care coordination
• Travel independently to patient homes; clients are grouped to minimize drive and windshield time, though occasional travel of 50+ miles may occur

Required Qualifications

• Active California SLP license required
• Basic Cardiac Life Support (BCLS) certification required
• Minimum of 1 year of recent skilled home health experience (at least 1 year within the last 3 years)
• Proficiency with HCHB EMR system required
• OASIS experience required
• Must have reliable personal transportation

Preferred Experience / Skills

• Experience working with geriatric patient populations
• Familiarity with home health productivity and point-based documentation standards
