Key Responsibilities

• Complete comprehensive evaluations of assigned patients and develop, implement, and modify individualized treatment plans in alignment with patient needs, goals, and physician direction
• Maintain clear, concise, and timely medical record documentation including goal writing, functional quality indicator assessments, letters of medical necessity, and charge entry in compliance with state, federal, and payer guidelines
• Ensure all care is provided under signed, valid physician orders and within insurance approvals as required
• Delegate and direct discipline-specific therapy services for assigned patients, designing functional treatment programs that support return to community activities
• Serve as an educator for patients, families, students, and coworkers; participate in student programs by providing supervision, developing learning objectives, and completing student evaluations
• Adhere to state Practice Act and all applicable regulatory requirements; maintain professional licensure in good standing and fulfill all continuing education obligations

Required Qualifications

• Active Physical Therapist license in good standing (Colorado licensure required)
• Demonstrated ability to perform comprehensive patient evaluations and develop evidence-based treatment plans
• Proficiency in clinical documentation and compliance with state, federal, and payer guidelines
• Commitment to patient and employee safety standards and safe patient handling practices

Preferred Experience / Skills

• Prior experience in an outpatient therapy setting
• Experience supervising or mentoring PT students
• Familiarity with performance improvement methodologies and quality initiatives
