Contract | Full-Time | Days (8:00 AM – 5:00 PM, 5x8-hour shifts) | Onsite | Up to 26 weeks | Weekend rotation every 4–6 weeks
Key Responsibilities
- Evaluate patient functional abilities and disabilities — including range of motion, strength, and coordination — through standardized assessments; establish functional baselines and monitor progress throughout treatment
- Interpret and implement referrals; develop and implement individualized education and treatment plans, including goal setting and estimating length of treatment
- Deliver direct patient care through individual and/or group Physical Therapy activities in accordance with the treatment plan, serving patients across Van Buren, Calhoun, and Kalamazoo Counties
- Participate in interdisciplinary team discussions, rounds, and clinics; communicate with the care team to coordinate patient care
- Document evaluations, treatment plans, treatments performed, and patient progress accurately and in compliance with required timelines within the electronic health record
- May design and fabricate splints and assistive devices; may cross-train and provide coverage in other areas within Rehabilitation Services as needed
Required Qualifications
- Valid, active Michigan Physical Therapist license in good standing required
- BLS certification through the American Heart Association (AHA) required
- Minimum of 1 year of Physical Therapy experience required; home health experience is required
- Valid, active driver's license in good standing; daily access to a personal vehicle for business use and proof of auto insurance required
- Supervisor-level references required
- Locals are welcome; no radius restrictions apply
Preferred Experience / Skills
- Experience working across multiple counties or in a community-based home health setting
- Familiarity with electronic health record documentation systems and compliance timelines
- Experience providing patient and family education in a home or community setting
